It has come to my attention that some computers are not printing the quilt entry document properly. Please pay close attention to these instructions and be sure your form prints to look exactly like the one below. I have added the PDF option and have instructions for printing from the photo. That is only as a last resort though. The final alternative is to fill out a form when you bring your quilts in for quilt collection on Friday, August 15, 9:30-3:00.

799 SW Columbia Street located adjacent to Riverbend Park.
Also, please note: ONLY USE WATERPROOF INK ON YOUR QUILT FORM. Check your pen and your printer ink.

Your Quilt Entry Form MUST look like this. If it has a different format it won't attach to the quilt properly. If you can't get either of the above forms to work, maybe you have a friend who can print for you. As a last resort, you can try printing from this photo. Here's how to do it. Click the photo, then your print button. On my computer I had to change the options to landscape format, remove the headers and footers, and no margin. Then you have to print just Page 1.
